Individual and Family Dental Plans

Primecare’s individual dental plans are packed with features and as a stand-alone solution can be purchased independent of any other benefit plan.

These plans provide affordable care through a network of highly qualified dentists. There are no waiting periods, calendar year maximums, or deductibles and no claim forms. Each enrolled family member may choose their own dentist from the Primecare directory, and the plans are comprehensive, covering more procedures than many other dental benefits plans available. These plans are perfect for the self-employed, part-time employee, or those without an opportunity to participate in any other dental benefit program such as seniors and cash paying consumers.

 

Employer Group and Association Dental Plans

Primecare’s employer group dental plans emphasize preventive care and early dental disease detection. Employees receive greater benefits at a lower premium from credentialed network dentists with no set plan maximums, deductibles or claim forms.

Primecare’s group plans are offered on an employer paid or voluntary basis without contributing to its cost. Employers can choose to pay any, or the entire low monthly premium. There are minimal participation requirements so employers don’t have to worry if only a few employees enroll. There are no pre-existing condition limitations. No employee will be refused coverage. Primecare offers a wide range of managed care dental plans covering all general and specialist dentistry needs for employer groups of all sizes.

What is Primecare?

Primecare contracts with established dentists throughout California to deliver quality dental care to plan members at reduced rates. Primecare offers several different dental plans to fill almost everyone's needs. Many services are provided at no additional cost to the member.

In addition to substantial savings, there are many other advantages to a Primecare dental plan. There are no waiting periods for treatment. Primecare plans have no annual deductible to be met and you never have to worry about a maximum limit of coverage.

What charges will I pay other than the premium?

For dental treatment, you will pay directly to your assigned Primecare contracting provider the co-payments listed on the Schedule of covered Service Payments for the plan that you choose. For procedures not covered by your plan, or for treatment by a non- Primecare provider, including specialists, you will be charged the usual and customary fees of those dentists.

May my family be covered?

You may extend coverage to your spouse, your unmarried children until age 19, your unmarried children who are full-time students until age 23, and mentally or physically challenged dependents living at home.

Where do I receive my dental care?

Primecare is very particular about its dental care providers, so not every dental office qualifies. You may choose the office most convenient for you from Primecare's list of participating dental care providers. If you ever want to change offices, just send us a note and tell us to which office you want to switch. We do the rest.

What about treatment by a dental specialist?

Primecare dental covers all dental specialty treatments including implants, cosmetics, sedation, orthodontics, and much more.

Does Primecare have a plan for seniors?

Yes. Both you and your spouse are eligible for Primecare's Senior Savings Premium which entitles you to reduced rates on our non-group plans.

Does Primecare cover braces?

Yes. Primecare Dental Plan provides complete orthodontic coverage.

What if I have an emergency while I'm away from home?

If there is no Primecare provider within 30 road miles, you may seek emergency treatment at any dental office. Primecare will reimburse you up to $100 upon presentation of a receipt or statement showing the emergency treatment received by you or your family member.

How may I enroll?

It's easy. Complete the Primecare Enrollment Application enclosed with this information. Select the plan that you desire, decide on the most convenient method of premium payment, and choose the desired Primecare contracting provider office most convenient for you. Mail the forms directly to Primecare together with the appropriate first premium payment.

When will my coverage start?

For applications received prior to the 25th of the month, coverage will begin on the first of the following month.
