PrimeCare Dental is a Knox-Keene licensed Dental Health Maintenance Organization, offering a variety of prepaid and discount plans throughout California.
For dental treatment, you will pay directly to your assigned PrimeCare contracted provider the co-payments listed on the Benefits Schedule for the plan that you choose. For procedures not covered by your plan, or for treatment by a non-PrimeCare provider, including specialists, you will be charged the usual and customary fees of those dentists.
You may extend coverage to your spouse, your unmarried children until age 26, and mentally or physically challenged dependents living at home.
You may receive dental care from the assigned dental office you choose when you submit your application. You can always choose a different office throughout the term of your membership. To do so, please contact a Member Service Representative at 1-800-937-3400.

Yes. Both you and your spouse are eligible for PrimeCare Dental’s senior plan which entitles you to reduced rates on our non-group plans. Please click here to enroll.
Many PrimeCare Dental plans provide orthodontic coverage. Please refer to your Evidence of Coverage or contact a Member Service Agent at 1-800-937-3400, for more assistance.
If there is no PrimeCare provider within 30 road miles, you may seek emergency treatment at any dental office. PrimeCare will reimburse you up to $50 upon presentation of a receipt or statement showing the emergency treatment received by you or your covered dependent. You may download and complete the paper application, and mail with payment to PrimeCare Dental Plan. If you require further assistance, you may contact a Member Service Representative at 1-800-937-3400.
For applications received prior to the 25th of the month, coverage will begin on the first of the following month. For immediate coverage, please enter the 1st day of the current month as your effective date.
